Long-Term Care: Needed when your loved one is at a point in life when they are in need of assistance with activities of daily living. Cypress Creek provides rehabilitative, restorative, and ongoing skilled nursing care to residents.
Short-Term Care: Sometimes your loved one needs a place for a short time. Normally this is several weeks or a few months following a surgery, injury, illness, or other medical condition.
Respite Care: When the primary care giver needs time away for their own needs, Respite Care steps in to fill the gap. After a short 3-7 day stay in our facility the loved one heads back home.
Hospice Services: Our care team provides end of life medical, psychological, and spiritual support. The goal of hospice is to provide peace and comfort to an individual and their family.
Skilled Nursing Care: When a resident's need for care or treatment can only be done by licensed nurses such as wound dressings, rehabilitation, or rapidly changing health status, Cypress Creek has the correct skill set on site
Rehabilitation and Therapy: After a stroke, surgery, injury, or illness, our rehabilitation team will create a customized program taylored to meet your goals and get you back to your original health. Sum Valley offers rehabilitation services 7 days a week and provide physical, occupational, and speech therapy..
